Learning Objectives
3.1 · Communication as a Clinical Skill
Communication failure is one of the largest contributors to preventable harm in hospitals, which is why effective communication between caregivers is one of the six International Patient Safety Goals. It is a clinical skill that is taught, assessed and improved — not a personality trait some nurses happen to have.
It has measurable effects. Patients who understand their plan take their medicines more reliably, return to hospital less often, and complain less — and most complaints about clinical care turn out, on examination, to be complaints about communication rather than about the treatment given.
3.2 · The Therapeutic Relationship
The relationship is UNEQUAL IN POWER and THERAPEUTIC rather than SOCIAL — which is what makes boundaries necessary. The line falls at secrecy, off-duty contact, gifts beyond a token, and self-disclosure that serves you rather than him.
Termination is the phase most often skipped and it matters most to patients who have been in hospital a long time. Ending abruptly — transferring or discharging without a closing conversation — leaves the relationship unfinished and is experienced as abandonment even when the care was excellent. Some texts add a pre-interaction phase before the first meeting (preparing, reviewing the record, checking one's own feelings), and some use Peplau's four-phase wording (orientation, identification, exploitation, resolution); this chapter teaches the three-phase form.
The relationship is unequal in power and therapeutic rather than social, which is what makes boundaries necessary. Chapter 01, section 1.17 sets out where the line falls: secrecy, off-duty contact, gifts of value, and treating one patient as special.
3.3 · Non-Verbal Communication
Most of what a patient reads from you is NOT IN YOUR WORDS: posture, distance, eye contact, facial expression, touch, tone — and how much you appear to be IN A HURRY.
When words and behaviour disagree, the patient believes the behaviour. A patient who says he is fine while gripping the sheet, avoiding your eyes and answering in short sentences is telling you something, and naming the mismatch gently — "you say you're fine, but you seem uncomfortable" — is itself a therapeutic technique.
Non-verbal norms are culturally variable. Direct eye contact, personal distance, and touch between men and women carry different meanings in different communities, and what reads as warm attentiveness to one patient reads as intrusive to another.
3.4 · Therapeutic Techniques
Purposeful silence gives the patient room to think and to find words for something difficult. Offering self — "I'll sit with you for a while" — signals availability without demanding disclosure. Summarising closes a conversation by pulling the threads together and checking you heard correctly.
Focusing brings a wandering account back to what matters: "you mentioned the chest pain — can we come back to that?" Exploring goes deeper into something raised. Presenting reality gently corrects a misperception without arguing: "I don't hear the voices, but I understand that you do."
3.5 · Non-Therapeutic Responses
- ‘Don’t worry, everything will be fine’«
- It is NOT kindness
- It DISMISSES the fear
- It PROMISES what nobody can promise
- And it teaches the patient not to raise it again
- Name what you see: ‘you sound worried about tomorrow’
- Then STOP and let him fill the space
- Give honest information where you have itتُقدَّم معلومة صادقة حيثما توفّرت
- And say what you DO know: ‘I will be here, and I will stay with you’ويُقال ما هو معروف فعلاً: «سأكون هنا، وسأبقى معك»
- Which is a promise you can actually keep
"Why" questions demand a justification and put the patient on the defensive — ask "what happened?" or "tell me about it" instead. Giving advice takes the decision away from the patient. Changing the subject tells him the topic is unwelcome. Approving or disapproving makes your regard conditional on his choices.
Defensiveness — defending the ward when a patient complains — ends the complaint without addressing it. Minimising ("others have it worse") invalidates. Stereotyped responses ("everything happens for a reason") signal that you are not listening. And false agreement to avoid an awkward moment stores up a bigger problem later.
3.6 · Listening, Empathy and Sympathy
ACTIVE LISTENING is VISIBLE WORK: facing the patient, unhurried posture, not interrupting, following HIS direction rather than your checklist, and reflecting back what you heard. It is not silence while you wait for your turn to speak, and the difference is obvious from the outside.
- Understanding and NAMING the patient’s feeling
- FROM HIS position
- ‘This must be frightening.’«
- Keeps the focus ON HIMويبقى التركيز على المريض
- And it is what he needs
- Feeling SORRY, from YOUR position
- ‘I feel so sorry for you.’«أشعر بأسف شديد من أجلك»
- Moves the focus onto YOU and your feelings
- He then has to manage your distress as well as his ownفيضطر المريض حينها إلى التعامل مع ضيق مقدّم الرعاية بالإضافة إلى ضيقه هو
- Which is why it is the wrong answer on the exam
Empathy is understanding and naming the patient's feeling from his position: "this must be frightening." Sympathy is feeling sorry from your own position: "I feel so sorry for you." Empathy keeps the focus on him and keeps you useful; sympathy moves the focus to your feelings and often leads to reassurance you cannot back.
3.7 · Questions and Silence
- Gather BREADTH
- How you BEGIN
- ‘Tell me what has been happening’«
- Let him choose what matters
- Confirm a SPECIFIC fact
- How you NARROW DOWNكيفية تضييق النطاق
- But NEVER to identify a patient
- ‘Are you Mr Ahmed?’ is UNSAFE — a confused, deaf or compliant patient says yes«
- Ask him to STATE his name
SILENCE is an INTERVENTION, not a gap. After bad news, after a difficult disclosure, or when a patient is trying to find words, the useful response is TO STAY AND SAY NOTHING. Nurses fill silence because it is uncomfortable for THEM — and in filling it, they take away the moment the patient needed to speak.
3.8 · Anger, Demands and Distress
Anger is usually a SURFACE for something underneath — fear, pain, powerlessness, or a GENUINE FAILURE in the care given. The last of those is the one worth checking first.
Two things are never the answer: defending the ward before you have understood the complaint, and promising something you cannot deliver to end the confrontation. Both feel like resolution in the moment and both make the next conversation worse.
Safety comes first when anger escalates towards threat: keep the exit behind you, do not stand over the person or corner him, keep an arm's length, call for help early, and disengage rather than win. A patient who is threatening is a safety event, and it is reported.
K — Knowledge. Give a warning shot, then the news in plain language, in small pieces, pausing between them. E — Emotions. Respond to the reaction before adding information; this is where silence and empathy belong. S — Strategy and summary. Set out what happens next, so he leaves with a plan rather than only with the news.
The nurse's role is usually not to break the diagnosis — that sits with the treating physician — but to prepare the setting, be present during the conversation, and do the work afterwards: staying, answering what was not absorbed, and telling the team what the patient actually took in.
3.10 · Death, Dying and the Bereaved Family
News of a death is not given by telephone. When a family must be called, the message is that the patient's condition has changed seriously and that they should come in, with someone driving them if possible — and the death is told in person, in a private room, by someone who can stay. Being told on the phone leaves a family alone with it and travelling in distress. If a relative asks directly whether the patient has died, do not lie: say that this cannot be discussed properly over the telephone and that someone will meet them on arrival; if the question is pressed, answer plainly and stay on the line.
With a dying patient: hearing is widely held to persist late, so continue to speak to him, introduce yourself, and say what you are about to do. Answer honestly when asked how long — an honest "I don't know, but the changes we're seeing suggest it may be short" is better received than a confident number or an evasion. Do not promise there will be no pain; say what you will do about it.
With a bereaved family, presence outweighs words. Say you are sorry, use the person's name, allow silence and tears, and resist filling the space with explanation. Stock phrases — "he's at peace now", "at least he didn't suffer", "you must be strong" — are heard as dismissal. Religious and cultural practices around washing, shrouding and timing are part of the care, as Chapter 01 sets out.
3.11 · Hearing and Visual Impairment
- Get his ATTENTION firstيُلفَت انتباهه أولاً
- FACE him, mouth visible, LIGHT ON YOUR FACE — not behind youمواجهته، مع إبقاء الفم مرئياً والضوء على وجه المتحدّث لا خلفه
- Normal pace, slightly LOWERED pitch
- SHOUTING distorts the sounds and the face
- Rephrase rather than repeat the same words louder
- Hearing aid IN and switched ON; reduce background noise; write it downسمّاعة الأذن مركَّبة ومُشغَّلة؛ يُقلَّل الضجيج المحيط؛ ويُكتب ما يُراد قوله
- ANNOUNCE yourself on entering, and say when you are LEAVING
- Address him BY NAME so he knows you are speaking to himيُنادى باسمه ليعرف أنه المقصود بالحديث
- Describe what you are about to do BEFORE you touch himيُوصَف ما سيُفعل قبل لمسه
- Do not move his belongings without telling himلا تُنقَل أغراضه دون إخباره
- Orientate him to the room by the clock face
- Offer your arm rather than taking hisيُعرَض الذراع بدل الإمساك بذراعه
For a patient with visual impairment: announce yourself on entering and say when you are leaving, address him by name so he knows you are speaking to him, describe what you are about to do before you touch him, orient him to the room and keep furniture and his belongings exactly where they were, and offer your arm rather than taking his.
3.12 · Aphasia and Cognitive Impairment
- He UNDERSTANDS but cannot PRODUCE the words
- Give him TIME without finishing his sentences
- Questions he can answer with YES/NO or a gesture
- Offer a communication board
- And write instructions down — reading is often better preserved than speech (though the patient's own writing is usually limited too)وتُكتَب التعليمات — فالقراءة غالباً أفضل حفظاً من الكلام (مع أنّ كتابة المريض نفسه محدودة عادةً أيضاً)
- Approach from the FRONT; use his NAME
- ONE short instruction at a timeتعليمة قصيرة واحدة في كل مرة
- Allow LONG pauses for processing
- Do NOT test him with questions about date or place that only expose the deficit
- Do not argue with a false belief — respond to the feeling behind itلا يُجادَل في معتقد خاطئ — ويُستجاب للشعور الكامن وراءه
For a patient with dementia: approach from the front, use his name, one short instruction at a time, allow long pauses for processing, and do not test him with questions about date or place that only expose the deficit. When he says something factually wrong, respond to the feeling behind it rather than correcting the fact — arguing about reality raises distress and changes nothing.
3.13 · The Patient Who Cannot Respond
ASSUME THE PATIENT CAN HEAR. Sedation, unconsciousness and paralysis from a neuromuscular blocker do NOT reliably remove awareness, and patients regularly report afterwards what was said at their bedside.
Never talk across a patient as though he is absent. Handover, prognosis discussions and social conversation at the bedside are all heard, and being discussed in the third person while lying there is one of the experiences intensive care survivors describe most consistently.
For an intubated or tracheostomised patient who is awake, build a route out. Ask questions that need only yes or no, and agree a signal — a blink, a hand squeeze, a nod. Offer an alphabet board, a picture or symbol board, a writing pad if the hands allow, or a communication application. Establish a reliable call signal before he needs it, and give him more time than feels comfortable.
Frustration and fear are the dominant experiences of a patient who cannot speak, and the commonest failures are giving up after two attempts, guessing aloud until he stops correcting you, and letting the family answer for him. Record in the notes which method worked, so the next nurse does not begin again from nothing.
3.14 · Children and Older Adults
- Get DOWN TO EYE LEVEL
- Words matched to the DEVELOPMENTAL stage, not the chronological age
- Be HONEST about whether something will hurt — lying costs you every future procedure
- Allow choices where a REAL choice exists — which arm, not whetherتُتاح خيارات حيث يوجد خيار حقيقي فعلاً — أيّ ذراع، لا هل سيُجرى الإجراء أم لا
- Involve the parent, and let him hold the child
- Play and demonstration on a doll before the real thing
- NEVER assume hearing loss or confusion from AGE alone
- Allow more time for processing WITHOUT slowing into condescension
- Avoid diminutives and the plural ‘we’ — ‘shall we take our tablets?’
- Address him by the name HE uses
- Speak to HIM, not to the relative who came with himيُتحدَّث إليه هو، لا إلى القريب الذي رافقه
- Glasses and hearing aids IN before the conversation starts
With an older adult: never assume hearing loss or confusion from age alone, allow more time for processing without slowing your speech into condescension, avoid the diminutives and the plural "we" ("shall we take our tablets now?"), sit at eye level, and address the patient rather than the accompanying relative.
3.15 · Language Barriers and Interpreters
A QUALIFIED INTERPRETER is required whenever the patient cannot communicate reliably in the language being used — and ALWAYS for consent, diagnosis, discharge teaching, and anything with legal weight.
Working with an interpreter: speak to the patient, not to the interpreter, keep eye contact with the patient, use short segments and pause, use plain language without idiom, and check understanding with teach-back through the interpreter. Allow roughly twice the usual time, and record in the notes that an interpreter was used and who it was.
3.16 · Communicating in Saudi Practice
Family-centred communication is the norm and it does not remove the patient's autonomy. The workable approach is to ask the patient directly, early and privately, who he wants present in conversations and who he wants informed — then follow what he says rather than what the family assumes. That one question prevents most of the conflicts that follow.
Requests to withhold a diagnosis from the patient are common and are handled the same way everywhere: you may not lie to a patient who asks, but he may choose not to be told. Explore why the family is asking, find out from the patient how much he wants to know, and bring the treating physician into it — Chapter 01, section 1.1 covers the veracity rule.
3.17 · Patient-Centred Care and Shared Decisions
PATIENT-CENTRED CARE means care organised around THIS PATIENT’S values, preferences and expressed needs — rather than around the ward’s routine. In practice it shows up in small things: when meals arrive, when he washes, who is allowed to stay, and whether anyone asked.
- The team has CHOSEN a plan
- The patient ACCEPTS or REFUSES itالمريض يقبلها أو يرفضها
- One option on the table
- A legal requirement
- The REASONABLE OPTIONS go on the table
- WITH their benefits, harms and uncertainties
- Including the option of DOING NOTHING
- And the patient’s VALUES decide between themوقيم المريض هي ما يحسم بينها
- They are NOT the same thing — which is the examined point
Shared decision-making is not the same as informed consent. Consent asks the patient to accept or refuse a plan the team has chosen. Shared decision-making puts the reasonable options on the table with their benefits and harms, elicits what matters to the patient, and reaches a decision together. It is the right approach wherever there is genuine equipoise between options.
3.18 · Health Literacy and Plain Language
HEALTH LITERACY is the ability to OBTAIN, UNDERSTAND and ACT ON health information — and low health literacy is COMMON, INVISIBLE, and NOT reliably predicted by education level. A professor can have it.
- SHORT sentences; everyday words
- ONE idea at a timeفكرة واحدة في كل مرة
- Most important point FIRSTالفكرة الأهم أولاً
- No jargon without translating itلا مصطلحات دون ترجمتها
- ‘High blood pressure’ not hypertension; ‘water tablet’ not diuretic«
- Ask him to say WHAT HE WILL DOيُطلب منه أن يقول ماذا سيفعل
- Not ‘do you understand?’ — everyone says yes
- Frame it as a check on YOUR explaining
- ‘I want to be sure I explained this well — can you tell me how you’ll take it?’«أريد أن أتأكّد أنني شرحت هذا جيداً — هل يمكنك إخباري كيف ستأخذه؟»
- Which removes the shame and gets you an honest answer
Plain language: short sentences, everyday words, one idea at a time, the most important point first, and no jargon without translating it — "high blood pressure" rather than hypertension, "water tablet" rather than diuretic. Aim for three key points per session, not everything you know, and support words with a picture or a demonstration.
Confirm with teach-back, covered in Chapter 02: ask the patient to say what he will do, and frame it as a check on your own explaining rather than a test of him — "I want to be sure I explained this well; can you tell me how you'll take it?"
3.19 · Readiness, Barriers and the Domains of Learning
Before teaching anything, assess READINESS. A patient in severe pain, acutely breathless, sedated, frightened or newly bereaved CANNOT LEARN — and teaching delivered into that state is wasted, however well it is done.
Other barriers to check: pain, fatigue, sensory impairment, language, low literacy, anxiety, denial, cultural mismatch between the teaching and the patient’s life, and simply the wrong time of day. Most are removable, and removing one is usually faster than teaching harder.
Three domains of learning, each needing a different method and a different evaluation. Cognitive — knowing: taught by explanation, evaluated by the patient stating it back. Affective — attitudes and feelings: approached by discussion and exploring concerns, evaluated by what he expresses. Psychomotor — doing: taught by demonstration, evaluated only by return demonstration.
3.20 · Teaching Adults
A workable teaching session: agree what the patient wants to get from it, teach three points at most, use plain language and a demonstration, get a return demonstration or teach-back, give a written summary at a readable level, and schedule a short review rather than a single long session. Short and repeated beats long and once.
3.21 · Behaviour Change
- ARGUING for change
- Which reliably produces argument for the status quo
- Warning, lecturing, or prescribing a plan he did not ask forالتحذير أو الوعظ أو فرض خطّة لم يطلبها
- And the harder you push, the more he defends the behaviour
- Works WITH ambivalence instead of against itتعمل مع التردّد لا ضدّه
- OPEN questions
- AFFIRMATIONSتأكيدات إيجابية
- REFLECTIVE listening
- SUMMARIESتلخيص
- The patient argues himself into change — which is the only argument that holds
Match the response to the stage. Handing a smoking-cessation plan to someone in precontemplation produces resistance; the useful move there is to raise awareness and invite him to consider it. Detailed practical planning belongs in preparation, and relapse-prevention work belongs in maintenance.
Motivational interviewing works with ambivalence instead of arguing against it: open questions, affirmations, reflective listening and summaries. Arguing for change reliably produces argument against it — the patient defends the behaviour aloud and persuades himself in the process.
3.22 · Teaching Groups and Communities
Group teaching is NOT individual teaching repeated to more people. It is efficient, and it adds PEER LEARNING — patients accept from someone living with the same condition what they resist from a professional.
- Happens DURING the session
- And lets you ADJUST
- Questions back, a show of hands, WATCHING WHO IS LOST
- Its purpose is to change what you do next
- Happens at the END
- And measures whether the objectives were MET
- Return demonstration, a short quiz, a stated plan
- Its purpose is to judge the session, not to rescue itغرضه الحكم على الجلسة، لا إنقاذها
Community health education extends the same thinking beyond the ward: know WHO the audience is and WHAT THEY ALREADY BELIEVE; work through the settings and people they already TRUST — the mosque, the school, the family elder; match the message to the culture and the language; and evaluate by BEHAVIOUR, not by attendance.
Plan a session backwards from what people must be able to do at the end. Write two or three behavioural objectives, choose methods that match the domain — explanation for knowledge, discussion for attitudes, demonstration and practice for skills — allow time for questions, and finish with a summary and a written take-home at a plain reading level.
Evaluate at two levels. Formative evaluation happens during the session and lets you adjust — questions back, a show of hands, watching who is lost. Summative evaluation happens at the end and measures whether the objectives were met: a return demonstration for a skill, a short quiz for knowledge, and where possible an outcome measure weeks later, which is the only evidence that anything changed.
3.23 · Health Promotion and Levels of Prevention

HEALTH PROMOTION is BROADER than prevention: it is enabling people to increase CONTROL over their health, through education, environment and policy TOGETHER. Nursing contributes at every level — the immunisation given, the screening offered opportunistically, the rehabilitation supported, and the policy argued for.
The distinction that catches people: screening a well person is secondary prevention, not primary, because the disease may already be present — you are looking for it early rather than preventing it. And rehabilitation after a myocardial infarction is tertiary, even though it also prevents a second event.

Conflict handled well is a NORMAL part of team work. Address it EARLY and PRIVATELY; describe the BEHAVIOUR and its EFFECT rather than labelling the person; listen to the other account BEFORE responding; look for the shared goal — which is almost always the patient; and agree what will happen differently next time, because a conversation that ends without that changes nothing.
When a concern is not acted on, escalate using a graded approach: state the observation, state your concern, propose a solution, and if it is still not addressed, say plainly that you need a decision because the patient is at risk. Then go up the chain of command and document each step, as Chapter 01, section 1.18 sets out.
Conflict handled well is a normal part of team work. Address it early and privately, describe the behaviour and its effect rather than labelling the person, listen to the other account before responding, and separate the problem from the personality. Collaboration produces the best outcomes; avoidance works only for trivia; competing is for emergencies where someone must decide now.
3.25 · SBAR and Structured Clinical Communication٣٫٢٥ · SBAR والتواصل السريري المنظَّم
And the rule that runs through all of it: anything clinical taken VERBALLY is WRITTEN DOWN and READ BACK AT THE TIME — orders and critical results alike, while the other person is still on the line.
Two letters are routinely got wrong. A is Assessment — your clinical interpretation, the sentence beginning "I think" or "I'm concerned that" — and not address, allergies or anything else. And R is Recommendation, an explicit request with a timeframe. "I just wanted to let you know" is not a recommendation, and a call that ends without one usually has to be made again.
Some services use ISBAR, adding Identify at the front — your name, your role, your unit, and the patient's identity confirmed with two identifiers. It exists because calls in which nobody establishes who is speaking about whom are a recurring source of error.
Anything clinical taken verbally is written down and read back at the time — orders and critical results alike. Chapter 02 covers shift handover, transfer handover and verbal orders in full; the point here is that structure is what replaces the non-verbal information a telephone strips out.

Written information for patients should be at a plain reading level, in the patient's language, with the key action first, short lines, and pictures where a step is physical. It supplements teaching and never replaces it — a leaflet handed over at discharge with no conversation is not patient education.
In remote and telehealth contacts, confirm identity with two identifiers before anything clinical, confirm the patient is somewhere he can speak freely, be explicit about what you cannot assess without examining him, agree what would make him seek care in person, and document the encounter as fully as a face-to-face one.
